Statement Delivery Disclosure

Thank you for your interest in Electronic Account Statements (eStatements) and Disclosure Notices from Premier Bank! To complete your request to discontinue paper statements and obtain your periodic statements electronically, carefully read the agreement below and indicate your acceptance.

Authorization and Eligibility

You understand that by indicating your acceptance of the terms and conditions of this Consent and Authorization, you authorize Premier Bank to discontinue sending such periodic account statements and disclosure notices via postal mail service and to instead deliver such periodic account statements and disclosure notices electronically. Changes to the delivery method of your periodic statement and notices will be subject to review by Premier Bank. Provisions in other Premier Bank agreements and disclosures may be revised from time to time and remain effective for all other aspects of the accounts involved.

You further understand and agree that:

1. Accounts eligible for electronic statement delivery include checking, savings and money market accounts.
2. Your consent to receive electronic periodic statement delivery also includes, but is not limited to, electronic delivery of other notices and disclosures that Premier Bank may send with your account statement(s); such as, notices about changes in the terms to your account(s) or associated services, disclosures and notices required by regulation to be provided in writing, privacy notices, and notices about changes in fees or product pricing.
3. If the eligible account is part of a combined statement and is the primary account, the electronic statement will include those other accounts.
4. You are an authorized signer of the account(s) being accessed. Your electronic signature executed in conjunction with this Statement Delivery Disclosure shall be legally binding on all owners on the account including yourself.
5. If any Account Number specified within this Statement Delivery Disclosure is a joint account, you acknowledge that you are the primary accountholder of the joint account and that your consent and authorization has been authorized by each of the joint accountholders. All joint accountholders on the account are legally bound by the terms and conditions of this Statement Delivery Disclosure.

Notification of eStatement Availability

If you elect to receive electronic account statements and notices, you will be able to access electronic statements, once published, for 18 months. Statements are generally available to view through Premier Mobile Online on the 2^nd business day following the statement cut date. As part of your acceptance of the Statement Delivery Disclosure, you must maintain a valid, active email address and must promptly notify Premier Bank of any change to your email by contacting Premier Bank using one of the following methods.

1. Premier Mobile Online
2. Email [email protected]. Do not email private or confidential information as this email is not secure.
3. Call your local office or the main location at (651) 788-8020 or (800) 772-6497.

Hardware and Software Requirements

Electronic statements may be viewed in PDF format. For you to access, view, or print electronic statements, you must be enrolled in Premier Mobile Online, and your computer must meet the minimum requirements:

1. Internet access and a valid email address
2. A currently supported version of Adobe Acrobat Reader
3. A currently supported internet browser
4. Cookies must be enabled in the browser options
5. A printer or other device, such as a storage medium (hard drive) so that you are capable of printing and/or retaining Internet-based data
6. To retain and store electronic statements on your computer, you can save the statement in PDF format. For PDF retention and storage, your computer must have a currently supported version of Adobe Acrobat Reader.

How to Withdraw Consent or Receive Paper Copies

Your consent to receive account statements electronically is voluntary and may be withdrawn at any time. To withdraw your consent to receive electronic account statements, you may do so using one of the following methods. You understand that, upon termination, you will no longer have access to eStatements through Premier Mobile Online. Statements previously provided electronically through Premier Mobile Online will not be mailed to you upon termination; you must print them for your retention. There is no fee for copies of notices or disclosures. Fees may apply for the delivery of paper statements.

1. Go to Settings > Statement Delivery. Your access is based on your eligibility as the Primary Accountholder.
2. Email your request to [email protected] . Do not email private or confidential information as this email is not secure.
3. Call your local office or the main location at (651) 788-8020 or (800) 772-6497.
4. Send the request in writing to:

You may obtain a paper copy of your eStatement(s) at any time by printing it yourself. You may also request a paper copy of any periodic statement or disclosure notice from Premier Bank. Premier Bank will charge a fee, as disclosed in the applicable Account Fee Schedule, for a paper copy of the periodic statement(s). There is no fee to receive paper copies of notices or disclosures.

Important Notices

You understand and agree that Premier Bank reserves the right to change the terms and conditions of this Statement Delivery Disclosure. Premier Bank will notify you before the effective date of any change by mailing notice to you through the United States Post Office at the address currently on file for your account. Accessing your electronic account statement after you receive such notice will constitute your agreement to such changes.
